Our Chronic Disease Telemedicine Program is a chronic disease management program that aims to help you keep your chronic conditions under check.
The Program aims to make it easier for you to manage your chronic conditions at the comfort of your own home with regular teleconsults and chronic disease medication delivery.
Stable patients who are able to self monitor their blood pressure at home and are compliant to medication are suitable for the chronic disease telemedicine program.
You will get your chronic disease medication delivered the next working day.
Patients are allowed to use CHAS Chronic subsidies and/or MediSave for video-consultations for regular follow-up of conditions under the Chronic Disease Management Program (CDMP), if the video-consultation is conducted by a healthcare institution indicated as being CHAS/MediSave accredited on the Voluntary Listing of Direct Telemedicine Service Providers
Blood pressure machines are available at most pharmacy
The treatment differes based on the different chronic disease.
The Chronic Disease Telemedicine Program consists of the relevant lab tests, teleconsults and chronic disease medicine delivery.
The cost will depend on the individual’s conditions. Subsidies are available under CHAS, Medisave, Pioneer Generation and Merdeka Generation.
