What BMI Qualifies for Prescription Weight Loss in Singapore?

The short answer: a BMI of 30 kg/m² or above, or a BMI of 27 kg/m² or above if you have at least one weight-related health condition such as Type 2 Diabetes, high blood pressure, or dyslipidaemia. These are the internationally recognised thresholds used by doctors in Singapore to determine whether prescription weight loss treatment is medically appropriate.

If you fall below these thresholds and have no obesity-related conditions, lifestyle changes — diet and exercise — remain the recommended first-line approach.

What Is BMI and Why Does It Matter?

Body Mass Index (BMI) is calculated by dividing your weight in kilograms by your height in metres squared (kg/m²). It is the primary screening tool used to classify weight status:

It is worth noting that medication eligibility thresholds — derived from international prescribing criteria — use a slightly lower figure of 27 kg/m² when comorbidities are present. This means a patient with a BMI between 27.0 and 27.4 who has a condition such as hypertension or Type 2 Diabetes may still qualify for prescription treatment, even though they fall just below the local Asian obesity threshold. Your doctor will assess your individual risk profile to determine whether treatment is appropriate.

Who Qualifies for Prescription Weight Loss Treatment?

You are likely eligible if you meet either of the following criteria:

  • BMI ≥ 30 kg/m² — regardless of whether you have other medical conditions
  • BMI ≥ 27 kg/m² with at least one of the following: Type 2 Diabetes, hypertension, dyslipidaemia, obstructive sleep apnoea, or non-alcoholic fatty liver disease

You should also have made a reasonable attempt at lifestyle modification — diet changes and increased physical activity — without achieving sufficient weight loss.

Who Does Not Qualify?

Prescription weight loss treatment is generally not appropriate if you:

  • Have a BMI below 27 kg/m² with no weight-related health conditions
  • Are seeking weight loss for cosmetic reasons alone without medical indication
  • Have specific contraindications, including a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2 (relevant to GLP-1 medications)
  • Are pregnant or planning to conceive in the near term

What Medications Are Used?

The most commonly prescribed weight loss medications in Singapore are GLP-1 receptor agonists:

  • Semaglutide — administered once weekly
  • Liraglutide — administered once daily

Both work by suppressing appetite, slowing gastric emptying, and improving blood glucose regulation. They are most effective when combined with dietary changes and increased physical activity.

What Are the Risks and Limitations?

These medications are effective for many patients but come with important caveats:

  • Common side effects: nausea, vomiting, diarrhoea, and constipation — most prominent when starting or increasing the dose, and usually improve over time
  • Weight regain: the majority of patients regain a significant proportion of lost weight after stopping medication, meaning long-term or indefinite use may be necessary for sustained results
  • Variable response: not all patients achieve clinically meaningful weight loss; a trial period is typically used to assess response
  • Cost: GLP-1 medications are not subsidised under standard public healthcare pathways in Singapore and represent a significant ongoing expense (see below)

The fees for weight loss management would depend on the type of programme (exercise sessions/ health coaching) and the type of medication prescribed.

It may range from a $300 to $1400 per month.

  1. Record your height, weight, and calculate your BMI
  2. Take a medical history and assess for obesity-related conditions
  3. Review previous weight loss attempts

There is no fixed duration — this is typically a long-term treatment. Most patients use these medications for at least 12 months; many continue indefinitely given the risk of weight regain on stopping. Treatment duration is guided by individual goals, response, and ongoing medical review.

Can I stop the medication once I reach my target weight?

You can, but most patients experience weight regain after stopping. Your doctor will discuss a long-term maintenance plan with you.

Is prescription weight loss suitable for seniors?

It can be, depending on overall health status, kidney function, and existing medications. Older adults may need more careful dose titration and monitoring.

Are these medications the same as diabetes medications?
